报名准备

（1）户口本原件及户口本（户主、父母页及幼儿页）复印件。

（2）幼儿身份证明原件及复印件（护照或出生证）

（3）父母一方身份证（正反）复印件。

（4）疫苗接种登记本（小绿本原件和每页复印件）。

（5）儿童保健卡（原件和每页复印件）。

（6）《上海黄浦紫綦托育园入园信息登记表》（线上填写）

注：如登记为地段生，请于报名当日携带产证或租赁合同原件，无需复印。

Registration materials:

(1) The original of the household registration and the copy of the household registration (the head of household, the parents'page and the children's page).

(2) Original and photocopy of birth certificate for children

(3) Copies of one parent's identity card (both sides).

(4) Registration of vaccination (original and copy of each page).

(5) Physical examination card (original and copy of each page).

(6) Shanghai Huangpu Ziqi Kindergarten Entry Information Registration Form (fill in online)

璞子园（上海·紫霞路校区）位于上海市黄浦区紫霞路 115~117 号 1 层，是上海的核心区域之一，步行 5 分钟即可到达江边，地理位置十分优越。

校区目前已开设针对 6 至 18 个月婴儿的 Nido 课程，18 个月至 3 岁幼儿的 Toddler 课程。

Guidepost Montessori at Zixiaroad is located on Floor 1, No.115-117, Zixia Road, Huangpu District, Shanghai Huang'pu District at central Shanghai. It is one of the most popular areas of Shanghai. It is at an excellent location where it is only 5 minutes' walk to the Bund.

At Guidepost Zixia Road, Nido Program for infants aged 6 to 18 months, Toddler Program for children aged 18 months to 3 years are available.
